The following information is required (as applicable) and must be made available for the compliance audit. The confidentiality of all proprietary information will be respected. Any technologies and/or processes unique to your operation will not be disclosed.

All employees’ labor contracts

·          所有員工的人事檔案（如入廠登記表、身份證復(fù)印件等）、請(qǐng)假/離職記錄

All employees’ personal files, factory entry registration forms, photocopy of identity cards, applications for leave, termination records

·          工廠規(guī)章制度以及員工獎(jiǎng)懲記錄

Facility regulations, rules and records of bonus, penalties for all employees

·          社會(huì)保險(xiǎn)繳費(fèi)憑證，以及相關(guān)批文（如適用）

Social Insurance funds distribution records, and relevant waivers (if applicable)

Other approval or permission authorized by governmental departments (such as Comprehensive Working Hours System or other types of overtime waivers)

Production equipment list, records for inspection and maintenance of special equipment, annual inspection records and use permits for all types of special equipment (such as annual elevator and boiler safety inspection records), permits for employees engaged in the use of special equipment (such as forklift operators’ licenses, electrician’s licenses)

Fire drill, training records, and fire equipment inspection records

Records of subcontractors, such as the contact information and production process records

Business license, National Tax and Land Tax Registration, Completion of Construction Proof of Inspection

·          衛(wèi)生許可證, 廚工健康證、未成年員工體檢記錄及上崗登記證等(如適用)

kitchen Sanitation Permit, kitchen workers’ Health Certificates, underage workers’ Health Certificates and Work Registration (if applicable)

New employee and position-based health and safety training records, records of special training for employees engaged in chemical use

Occupational injury and accident log, certification of employee first aid training

·          排污許可證, 有害（危險(xiǎn)）廢物排放許可證， 化學(xué)品列表，化學(xué)品之物質(zhì)安全資料表，廢物處理程序文件(如適用)

Sewage discharge permission, toxic (dangerous) waste discharge permission

·          各個(gè)崗位風(fēng)險(xiǎn)及所應(yīng)提供之個(gè)人保護(hù)用品之評(píng)估(如適用)

Risk/hazard assessment of each workstation and assessment of personal protective equipment (PPE) that should be provided for each production process

備注：此上所列文件為評(píng)估所需之主要文件，如評(píng)估當(dāng)日需其它文件作參考，則視評(píng)估當(dāng)日具體情況而定。

Notes: additional documentation may be required depends on the situations on the day of the assessment.
